Universal Assessment and Expansion of Home Visitation

Visiting Nurse Association, Lutheran Social Services and Genesis Medical Center have a long history of offering home visitation services to new parents. Together they have developed Cornerstone, a prevention program designed to promote healthy, safe, self-sufficient families. This voluntary, free program helps parents with the transition into parenthood. Services include a hospital/home family assessment, health promotion visits by a nurse, and an intensive home visitation program (using trained family support workers). Participation can begin during pregnancy if needed and may last up to five years. Family support, education on parenting techniques, care coordination and linking of community resources are key to promoting the health and well-being of children.
